Drafted as the seventh pick in the 2011 NFL Draft, Smith had an immediate impact with the 49ers. In 2011, he recorded 14 sacks despite not being a starter, earning the Defensive Rookie of the Year award. By the following season, he became a starter, amassing 19.5 sacks, three forced fumbles, an interception, and 18 tackles for loss. His performance helped the 49ers reach the Super Bowl and earned him All-Pro honors.

Smith's career faced challenges after 2013, when he was involved in a DUI incident. He served multiple suspensions, including a nine-game suspension for violating the NFL's substance abuse policies. Released by the 49ers in August 2015 after a third DUI arrest, he spent time with the Raiders and had a brief but impactful return with the Dallas Cowboys in 2020, tallying five sacks and recovering two fumbles in 16 games.
